Reimbursement Process For Out of Network Vision Provider:
Contact Member Services at 800.877.7195 for help submitting a claim online or by mail. You don’t need to fill out a claim form when you see a VSP network eye doctor or provider. The doctor or provider will submit the claim directly to VSP for processing after your appointment. The doctor or provider will discuss any copays or out-of-pocket expenses with you during your appointment.
If you choose to see an out-of-network provider, your coverage will likely be less than when you see an VSP network eye doctor. To submit a claim, you will need a copy of the itemized receipts or service statements for each patient that includes the following information printed on them:

- doctor's name, office name, or name of website where purchased (ex: Warby Parker)
- name of patient
- date of service
- and each service received, and the amount paid
